2. Have you or your partner had sex with someone else?
3. Have you had direct skin-to-skin sexual contact with your partner?
4. Has the condom ever broken during sexual activity? 
5. Do you or your partner have body piercings or tattoos?
6. Have you or your partner ever injected drugs, including steroids?
7. Have you or your partner shared drug equipment (snorting straws/bills)?
8. Have you had sex while drunk or high?
9. Have you ever been sexually assaulted?
