Grade 7 students are eligible for free Hepatitis B vaccines and Meningococcal C. Public Health Nurses will be going to your child’s school this fall to administer these vaccines.

If you would like your child to receive these vaccines, follow the instructions below:
• Read both sides of the consent forms. 

• Review your child’s yellow immunization card. If your child has received Meningococcal C or Hepatitis B vaccine, complete the bottom of the consent form.

• If you would like your child to receive the vaccines at school, complete the top section of the consent form.

• Make sure the legal guardian for the child signs the consent forms.

• Return the forms to your child’s teacher as soon as possible.
