The Human Papillomavirus (HPV) is a very common virus that causes cervical cancer and genital warts. About 70 % of adults will have at least one HPV infection in their lifetime.
Grade 8 girls will be offered the free, publicly funded vaccine at school based clinics. If your child won’t be able to attend the clinic date at her school, there will be ‘catch-up’ clinics at the Health Unit. Before signing and returning this consent form, be sure that you review the information in the FAQ section. You may also consider reviewing this information with your daughter.

Chaque année en Ontario, des centaines de femmes reçoivent un diagnostic de cancer du col de l’utérus et jusqu’à 140 femmes en meurent. Grâce au nouveau programme ontarien de vaccination contre le VPH destiné aux filles de 8e année, vous pouvez aider à protéger votre fille contre le cancer du col de l’utérus.
On offrira aux filles de 8e année le vaccin gratuit, subventionné par le gouvernement, lors de séances de vaccination à l’école. Si vous voulez que votre fille soit vaccinée par votre médecin, les trois doses vous coûteront entre 400 $ et 500 $. Si votre fille ne peut pas se présenter à la séance à son école à la date prévue, il y aura des séances de « rattrapage » à l’Unité sanitaire.
