Meningococcal Vaccine Program for Grade 9 and Older Students Form - 2010-2011 School year

Students who are entering Grade 9 (1996 birth cohort) and older are eligible for the Meningococcal C vaccine. Students can make an appointment for this vaccine by contacting the Vaccine program at 519-258-2146 ext 1222. Download a consent for Meningococcal C vaccine.
