Meningococcal Vaccine Program for Grade 7 Students Form - 2010-2011 School Year

Students in Grade 7 (1998 birth cohort) will be offered the Menactra (Meningococcal Types A, C, Y, W-135) vaccine in the spring of 2011. Students in Grade 8 (1997 birth cohort) who have not received this vaccine will be eligible also. If parents wish to have their child immunized prior to school based clinics please contact the Vaccine Program at 519-258-2146 ext 1222 for an appointment.
