Victorian Order of Nurses (VON) Windsor-Essex

Services Offered:

Foot Care Program and health related education services focus on promotion of healthy feet. Advanced trained nurses provide foot care services in a clinic setting or in the homes of homebound clients who are unable to attend a foot care clinic.

Home Help Program is designed to provide in-home help to the frail, elderly, and adults with physical disabilities to remain in their homes. Assistance includes: 

  • Light housekeeping, including floor mopping, dishes, vacuuming, and dusting.
  • Laundry services, including folding and putting away.
  • Light bathroom and/or kitchen clean-up.
  • Making beds, including changing linens.
  • Meal preparation.
  • Grocery shopping.


In-Home Visiting Nursing and Shift Program ensures that a medical/nursing plan of care is followed. Includes all levels and complexities of care. Palliative care span is directed toward optimal health and independence.

Let’s Talk Seniors Wellness Program is designed to maintain and enhance the health and independence of seniors through increasing knowledge and understanding of healthy living practices. One-on-one counseling and interactive group wellness sessions are offered at numerous city and county sites.

Meals on Wheels Meals Program offers nutritious and wholesome meals delivered daily and provides personal and social contact and security checks. Frozen meals available for weekends and holidays.

Supportive Housing Program enables the elderly to age in place by providing personal support, homemaking, and attendant service in a supportive housing environment. Windsor has two designated sites providing daily on-site assistance and 24-hour availability.

 

Council on Aging (Windsor-Essex County)

The mission of the Council on Aging is to act as a co-ordination, education, advocacy, and research body to enhance the quality of life of seniors in Windsor-Essex County, in order that each person has the opportunity to achieve full potential in enjoying life, participating in, and contributing to it.

  • Speakers are available for presentations, workshops, and various discussions on aging. The goal of these informative sessions is to help people develop a positive view of their own aging process, expose false and negative stereotypes regarding aging, and promote and encourage positive attitudes.
  • The Council on Aging also provides various services aimed toward seniors and increasing awareness on topics such as falls prevention.

 

Centres for Seniors Windsor

The Centres for Seniors Windsor is a non-profit charitable organization for adults over the age of 50. They provide recreational, educational, and social programs and activities, as well as community support services.

Some recreational activities include elderobics, people on weights, tai chi, yoga, zumba, dance classes, and wii bowling leagues. They also offer aquafit classes at Adie Knox pool two days a week. These programs are offered at two locations in Windsor: the McEwan Ave. Centre and the East Side Centre.

 

Community Care Access Centre (CCAC) Erie St. Clair

CCAC receives referrals from family members. Placement coordinators are available to answer questions or address concerns specific to long-term care options, provide assistance in placing an individual in long-term care, and provide information on rest homes. CCAC can also arrange for home care services and provide information about and referrals to other community and health care services in Windsor and Essex County.

 

Geriatric Assessment Program (GAP) Windsor Regional Hospital

The GAP at Windsor Regional Hospital:

  • Promotes independent living, to help delay or prevent seniors from being placed in long- term care facilities (e.g., nursing homes).
  • Serves adults 55 years of age and older, who are frail, at risk, or have complex medical problems.
  • Provides a full assessment, education, support, and advice.
  • Gives referrals via physicians, hospitals, community agencies, family members, caregivers, or seniors themselves.

Falls Prevention Programs

The Chronic Disease Management Program comprises a multitude of activities, provisions and programs to help support Windsor/Essex residents to live independently and safely in their homes.

Active Aging and Health Management Program

The Active Aging and Health Management Program is a seniors’ wellness program serving seniors (50 years +) living with Chronic Diseases, looking for assistance with the self management of their health. This program, operated by the Windsor Essex Community Health Centre features an inter-professional team providing holistic, comprehensive programming. Some of the services provided include: individualized fitness and health management plans, social work support, group exercise classes, pool rehabilitation/aquatic exercises, small group health promotion workshops, physiotherapy expertise and speech language pathology services.

Clinical Services

The Windsor Essex Community Health Centre boasts an inter-professional team of physicians, nurse practitioners, registered nurses, registered dietitian, occupational therapists, physiotherapists, social workers, chiropodists, speech language pathologists, massage therapists, pharmacists, health educators, and support staff. The Felix Avenue site provides the Windsor West community with access to a robust primary health care team, health promotion/disease prevention programs, a comprehensive diabetes program, and student education services. Referrals to community programs and services are also provided to clients and their families. Individuals with a postal code beginning with N9A, N9B, N9C or N8X are eligible to receive care at the Felix Ave. clinic.

Community Outreach

The inter-professional team is committed to providing programs and services to Windsor West clients and families in their homes and workplaces. At-home therapy visits, health promotion and prevention community information booths and seminars, and safety home assessments are examples of some of the services we provide for those who qualify.

Grab Bar Initiative

Our dedicated team is providing free comprehensive home safety assessments for qualified seniors who may require installation of grab bars in their home. The process is overseen by an experienced occupational therapist who orchestrates and supervises the installation process.
